Indications:

Management of complex partial seizures and simple or complex absence seizures

Prophylaxis of migraine headaches

Therapy for bipolar disorder

Treatment of acute bipolar depression

Dosage:

100mg/ml

10-15mg/kg/day IV divided 12hr infused over 1hr, maximum dose 60mg/kg/day, do not exceed 14 days

Contraindications:

Contraindicated in patients with known hypersensitivity to the drug, in patients with known urea cycle disorders, in active liver disease and family history of liver dysfunctions and in porphyria

Mechanism of Action

Valproate is known to inhibit succinic semialdehyde dehydrogenase, this inhibition results in an increase succinic semialdehyde which acts as an inhibitor of GABA transaminase ultimately reducing GABA metabolism and increasing GABAergic neurotransmitter. As GABA is an inhibitory neurotransmitter, this increase results in increased inhibitory activity.
